Gateways to Opportunity Participant Applications

Gateways to Opportunity offers several programs and services for you. If you are applying for the Gateways Registry only, apply online for a quick and easy membership option. Follow the steps below to download one PDF for the program(s) you want to apply for or renew.

Step 1—SELECT one application below.

	Participant Application—Complete if you have not applied to a Gateways Program before.
	Information Update Form—Complete this form if you already have a Participant Application on file, are renewing your current participation, are a Registry-Member, or have participated in a Gateways Program before.

Step 2—CHOOSE supplement applications to participate in one or more programs.

	Credentials—Offers the Illinois Director, Level 1 ECE, Infant Toddler, and ECE Credentials.
	Registry Member—Offers an online tracking system, to track your education and training.
	Registry Trainer/Trainings—Offers the opportunity for you to become a Gateways Registry-approved trainer and submit a training.
	Scholarship Program—Offers scholarship money to return to school.
	Great START—Offers a wage supplement for already earned education and commitment.

Step 3—DOWNLOAD your application.

Simply click below and print this application on white paper. Fill it in using blue or black ink and return with all required documentation (a checklist is provided in the application to help).
